Local anesthetic maximum dose

Weight-based maximums for a single agent, or additive mixture math across agents. Toxicity is treated as additive, so a mixture does not raise the ceiling.

kg

Agent 1

%

1 for 1%, 0.25 for 0.25%

mL
Enter a weight and at least one agent with a concentration and volume to see the combined percent of maximum.

Maximums are conservative guides based on actual body weight; consider lean body weight in obesity. Epinephrine-containing solutions raise the threshold for most agents.

Formula

  • Dose (mg) = volume (mL) x concentration (%) x 10
  • Maximum (mg) = min(mg/kg x weight, absolute cap)
  • Combined = sum of each agent's dose / its own maximum; keep at or below 1.0

Source

  • Classic maximums per standard references (Miller's Anesthesia; NYSORA; package inserts). Values vary by source; these are the commonly cited conservative figures.
